I have forgotten my login information. What do I do?

You can retrieve it through your saved passwords on your browser or by clicking “Forgot Username“ or “Forgot Password“ below the CVS MyChart login fields. The system will guide you through the steps to recover or reset your information.

To retrieve your username, you’ll need to enter your full name and date of birth.
To reset your password, you’ll need to enter your username, date of birth, and ZIP code.
